Summary

Transvaginal natural orifice transluminal endoscopic surgery (NOTES) colectomy is a safe and feasible approach for treating sigmoid volvulus. This single-port transvaginal technique offers a minimally invasive option for colonic resection.

Low-Cost Single-Port (LoCoSP) Device for a Transcervical Approach in Minimally Invasive Transhiatal Esophagectomy

Background:

  • Natural orifice transluminal endoscopic surgery (NOTES) adoption is growing.
  • Many published NOTES procedures are hybrid, incorporating traditional surgical techniques.
  • Colonic resections via NOTES have predominantly utilized hybrid approaches.

Observation:

  • An 84-year-old female with symptomatic sigmoid colon volvulus underwent endoscopic detorsion and desufflation.
  • A complete sigmoid resection was performed using a single-port device entirely through a transvaginal route.
  • The procedure was completed in 135 minutes with no intraoperative or postoperative complications.

Findings:

  • The patient required minimal narcotic analgesia post-procedure.
  • Oral intake was resumed on postoperative day 3.
  • At 2-month follow-up, no complications or recurrent volvulus were noted.

Implications:

  • Transvaginal NOTES sigmoid colectomy is a viable and safe alternative for sigmoid volvulus.
  • This approach demonstrates the potential for NOTES in complete colonic resections.
  • Further research into transvaginal NOTES for colonic surgery is warranted.
