Summary

Background:

Long wait times for dermatology appointments pose a challenge in urban primary care.
Teledermatology offers a potential solution for improving access to dermatological consultations.

Purpose of the Study:

To evaluate the accuracy, access time, cost, and acceptance of asynchronous teledermatology referrals.
To assess the feasibility of teledermatology in an urban primary care setting.

Main Methods:

Primary care physicians referred patients for asynchronous teledermatology consults.
Nurse-collected patient history and digital images were electronically sent to a dermatologist.
Fifty-two patients completed the study, with data analyzed for accuracy, access time, cost, and acceptance.

Main Results:

Teledermatology diagnosis accuracy was 92% overall (97% with adequate images).
Access time for teledermatology opinion was 1.9 days versus 52 days for in-person appointments.
Teledermatology was favored by patients and physicians, with cost savings noted compared to in-person referrals.

Conclusions:

Asynchronous teledermatology is an accurate and efficient tool for triaging dermatology referrals.
It significantly reduces patient waiting times and offers a cost-effective alternative to in-person consultations.
Teledermatology can enhance dermatological care delivery in urban primary care practices.
