  • Latissimus dorsi flaps with bony components were used for mandibular reconstruction.
  • Main Results:

    • Sixteen patients underwent reconstruction with no flap loss.
    • Four patients experienced limited shoulder range of motion; eight required re-intervention for postoperative complications.
    • Fourteen patients resumed oral nutrition, and speech returned to normal in most.
    • Satisfactory aesthetic results were noted post-atrophy of the latissimus dorsi muscle.

    Conclusions:

    • Subscapular system free-tissue transfer is a safe and reliable technique for extensive oro-facial defects.
    • This method provides satisfactory functional and aesthetic results in mandibular reconstruction.
    • It is a viable option for complex head and neck cancer defects.
