Summary

Children with hematologic disorders undergoing tonsillectomy may not face a higher risk of post-operative hemorrhage. Proper pre-operative evaluation and treatment appear to mitigate bleeding risks in these pediatric patients.

Background:

  • Tonsillectomy is a common pediatric surgical procedure.
  • Post-operative hemorrhage is a significant complication.
  • The risk of hemorrhage in children with hematologic abnormalities is not well-established.

Purpose of the Study:

  • To investigate the risk of post-operative hemorrhage in children with hematologic disorders undergoing tonsillectomy.
  • To determine if hematologic abnormalities increase the likelihood of bleeding after tonsillectomy.

Main Methods:

  • Retrospective chart review at a tertiary referral hospital.
  • Inclusion of all pediatric patients undergoing tonsillectomy with or without adenoidectomy over two years.
  • Comparison of hemorrhage rates between children with and without hematologic disorders.

Main Results:

  • A total of 462 patients underwent tonsillectomy.
  • Fourteen patients had identified hematologic abnormalities.
  • Only one patient (1/14) with a hematologic abnormality experienced post-tonsillectomy hemorrhage; this patient received pre-operative medical treatment.
  • Patients with abnormal lab values treated pre-operatively did not experience hemorrhage.

Conclusions:

  • Children with hematologic abnormalities may not have an increased risk of post-tonsillectomy hemorrhage.
  • Appropriate pre-operative evaluation and medical correction of coagulation disorders are crucial.
  • These findings suggest that optimized management can reduce bleeding complications in this patient group.
