Background:

  • Tonsilloliths are tonsillar biofilms causing halitosis, foreign body sensation, and sore throats.
  • Persistent tonsilloliths with cryptic infections often necessitate treatment.

Purpose of the Study:

  • To evaluate Laser Tonsil Cryptolysis (LTC) as an office-based alternative to tonsillectomy for persistent tonsilloliths.
  • To assess complications, patient satisfaction, and conversion rates to tonsillectomy.

Main Methods:

  • Retrospective chart analysis of 500 consecutive Laser Tonsil Cryptolysis (LTC) procedures.
  • Review of complications including bleeding, need for additional procedures, patient satisfaction, and conversion to tonsillectomy.
  • Procedures performed in-office under local anesthesia using CO2 or diode lasers.

Main Results:

  • Laser Tonsil Cryptolysis (LTC) showed a low complication rate, with bleeding in 6 patients and 1.16 sessions per patient on average.
  • Only 3.6% of patients required conversion to complete tonsillectomy.
  • High patient satisfaction was reported with minimal work absence (0-2 days).

Conclusions:

  • Laser Tonsil Cryptolysis (LTC) is a viable alternative to tonsillectomy for selected patients with tonsilloliths and cryptic tonsillitis.
  • Benefits include avoidance of general anesthesia, reduced pain, shorter recovery, and cost-effectiveness.
  • Suitable for patients with small tonsils, controllable gag reflex, and cooperative adults.
