Routine upper GI endoscopy before bariatric surgery is often unnecessary. Most findings were mild and did not alter surgical plans, suggesting it may not be needed before gastric bypass.
Area of Science:
- Gastroenterology
- Bariatric Surgery
- Endoscopy
Background:
- Routine upper GI endoscopy (UGI) is standard before bariatric surgery in Finland.
- UGI is invasive and resource-intensive.
- Helicobacter pylori (HP) testing from blood negates UGI as a sole indication.
Purpose of the Study:
- To identify common findings during preoperative UGI.
- To determine if UGI findings impact surgical decisions or lead to cancellations.
Main Methods:
- Retrospective analysis of 412 patients undergoing preoperative UGI.
- Data collected from Vaasa Central Hospital between 2006-2010.
Main Results:
- 55.8% of UGIs were normal.
- Common findings: hiatal hernia (25.4%), gastritis (13.7%), esophagitis (13.2%).
- Benign polyps (6.7%) and ulcers (2.9%) were found; no malignancies. HP prevalence was 12.0%.
Conclusions:
- Preoperative UGI findings were generally mild and benign.
- Findings did not influence operative plans.
- Routine UGI is not supported before gastric bypass due to low impact on surgical decisions.
