  • A 15-year-old male presented with a 4-month history of right eye proptosis and inferior displacement.
  • Imaging revealed a large (3.5 × 2 cm) cystic lesion in the superior orbit.

Findings:

  • Surgical resection via transconjunctival orbitotomy was performed.
  • Histopathology confirmed a cyst with nonkeratinized epithelium and absence of goblet cells.
  • No recurrence was observed during a 10-year follow-up period.

Implications:

  • Giant conjunctival primary orbital cysts, though rare, require accurate diagnosis via imaging.
  • Surgical intervention is effective, with potential for long-term positive outcomes.
  • This case contributes to understanding the management of uncommon orbital tumors.
