Rapidly dividing tumors, embryos, and wounded tissues require more oxygen than usual, lowering the oxygen concentration in the blood. Blood vessel formation starts early during embryonic development, around day 7. In the extraembryonic yolk sac, mesodermal precursor cells called hemangioblast proliferate and differentiate into angioblast. Angioblasts express vascular endothelial growth factor receptor 2 or VEGFR2, which binds VEGF-A, a proangiogenic factor, guiding blood vessel formation.     Summary

    Systemic sclerosis patients with the late nailfold videocapillaroscopy (NVC) pattern show reduced endothelial progenitor cells (EPCs) and elevated vascular endothelial growth factor (VEGF). These findings suggest a role for VEGF and EPCs in SSc endothelial dysfunction.

    Background:

    • Systemic sclerosis (SSc) is characterized by vascular abnormalities.
    • Nailfold videocapillaroscopy (NVC) identifies microvascular changes in SSc.
    • The relationship between NVC patterns and angiogenic factors is not fully understood.

    Purpose of the Study:

    • To investigate the association between nailfold videocapillaroscopy (NVC) patterns and serum levels of angiogenic factors in patients with systemic sclerosis (SSc).

    Main Methods:

    • Peripheral blood samples from 60 SSc patients were analyzed for circulating endothelial progenitor cells (EPCs) and endothelial cells (CECs).
    • Serum levels of eight endothelial markers were measured, with validation in a separate cohort of 43 SSc patients.
    • NVC patterns were assessed by blinded investigators.

    Main Results:

    • Patients with the late NVC pattern exhibited significantly lower EPC levels and higher vascular endothelial growth factor (VEGF) levels.
    • These associations were confirmed in an independent replication cohort.
    • Multivariate analysis identified lower EPC counts, higher VEGF levels, digital ulcer history, and elevated modified Rodnan skin score as independent predictors of the late NVC pattern.

    Conclusions:

    • The study demonstrates a correlation between the late NVC pattern in SSc and decreased EPC counts and increased VEGF levels.
    • These findings highlight the potential involvement of VEGF and EPCs in the pathophysiology of endothelial injury and repair in SSc.
    • Further research is warranted to elucidate the specific roles of VEGF and EPCs in SSc vascular complications.
