Background:

  • Compartment syndrome of the leg can lead to ischemic contractures, including debilitating claw toe deformities.
  • While acute management is well-documented, optimal surgical strategies for established claw toe deformity remain less explored.

Observation:

  • A case of left great toe clawing secondary to ischemia following compartment syndrome is presented.
  • The deformity manifested as significant functional impairment of the great toe.

Findings:

  • Surgical intervention involved lengthening the extensor hallucis longus tendon.
  • Transfer of the extensor hallucis brevis tendon to the extensor hallucis capsularis was performed.
  • Percutaneous pinning of the great toe stabilized the correction.
