Summary

Sleep-disordered breathing is common in post-polio syndrome, causing poor sleep quality and respiratory issues. Non-invasive ventilation, like nasal CPAP or mask ventilation, can significantly improve sleep and daytime symptoms in these patients.

Background:

  • Post-polio syndrome (PPS) can lead to late-onset neuromuscular and respiratory problems.
  • Breathing disorders during sleep are hypothesized to contribute to PPS symptoms.

Purpose of the Study:

  • To investigate sleep-disordered breathing in post-polio patients.
  • To evaluate the effectiveness of non-invasive ventilation in managing these disorders.

Main Methods:

  • Polysomnography was performed on 13 PPS patients, divided into two groups: those on ventilatory assistance (rocking beds) and those without.
  • Patients requiring treatment were evaluated using nasal CPAP or nasal mask ventilation.

Main Results:

  • Patients on rocking beds had poor sleep quality and significant oxygen desaturation, improving with nasal mask ventilation but not CPAP.
  • A subset of patients without assistance (group 2b) had sleep disturbances and responded well to nasal CPAP for obstructive/mixed apnea.
  • One patient with mixed apnea and hypoventilation showed dramatic improvement with nasal mask ventilation.

Conclusions:

  • Sleep studies are crucial for PPS patients with daytime sleepiness and respiratory complaints.
  • Nasal CPAP is effective for obstructive and mixed apnea in PPS.
  • Nasal mask ventilation is beneficial for hypoventilation syndrome and severe sleep apnea in PPS, including those on rocking beds.
