Summary

A rare case shows colovesical fistula, a connection between the colon and bladder, can cause epididymitis (testicular inflammation). Colovesical fistulae are also the primary cause of pneumaturia (air in urine).

  • A 75-year-old male presented with acute left-sided epididymitis and testicular pain.
  • The patient reported a history of conservatively managed localized diverticular perforation.
  • He also reported passing air in his urine (pneumaturia).

Findings:

  • Ultrasound confirmed bacterial epididymitis, treated with IV antibiotics.
  • CT imaging revealed a colovesical fistula and air within the bladder.
