Nonsense-mediated mRNA Decay

The Upf proteins that carry out nonsense-mediated decay (NMD) are found in all eukaryotic organisms, including humans. Each protein has an individual role, but they need to work in collaboration. Background:

  • Non-alcoholic fatty liver disease (NAFLD) is prevalent in morbidly obese individuals.
  • Fetuin-A, a pro-inflammatory protein, plays a role in metabolic dysfunction.
  • The impact of bariatric surgery (BAS) on fetuin-A in NAFLD patients requires further investigation.

Purpose of the Study:

  • To prospectively evaluate the effects of weight loss following BAS on metabolic factors in morbidly obese patients with NAFLD.
  • To investigate changes in fetuin-A expression and hepatocyte apoptosis after BAS.
  • To compare outcomes between patients with NAFLD and non-alcoholic steatohepatitis (NASH).

Main Methods:

  • Prospective study of 108 morbidly obese NAFLD patients undergoing BAS.
  • Measurement of hepatic fetuin-A (mRNA, protein) via qPCR, Western blot, and immunohistochemistry.
  • Quantification of hepatocyte apoptosis (M30) and plasma adiponectin/fetuin-A levels (ELISA) at baseline and 4 weeks/6 months post-surgery.

Main Results:

  • Significant BMI reduction observed in both NAFLD and NASH groups 6 months post-surgery (P<0.0001).
  • Decreased hepatocyte apoptosis (M30/M65 levels) over 6 months post-BAS.

Conclusions:

  • Bariatric surgery demonstrates beneficial effects on NAFLD, evidenced by reduced hepatocyte apoptosis and improved adipokine profiles.
  • Fetuin-A expression is more prominent in NASH patients and its plasma levels are altered post-surgery.
  • Weight loss induced by BAS impacts fetuin-A dynamics, particularly in NASH, warranting further research into its role.
