Summary

Accelerated partial breast irradiation (APBI) offers an alternative to whole-breast irradiation (WBI) for early-stage breast cancer, showing excellent local control and cosmetic outcomes. APBI may improve treatment accessibility by reducing time and volume.

Area of Science:

  • Oncology
  • Radiation Oncology
  • Surgical Oncology

Background:

  • Whole-breast irradiation (WBI) is standard adjuvant therapy post-breast-conserving surgery.
  • Accelerated partial breast irradiation (APBI) is an emerging alternative for select early-stage breast cancer patients.
  • APBI offers reduced treatment volume and duration compared to WBI.

Purpose of the Study:

  • To review available modalities for APBI delivery.
  • To outline patient selection criteria and consensus guidelines for APBI.
  • To discuss current controversies and ongoing trials comparing APBI with WBI.

Main Methods:

  • Review of early trials and ongoing phase III trials comparing APBI and WBI.
  • Analysis of APBI delivery modalities.

  • Evaluation of patient selection criteria and guideline recommendations.
  • Main Results:

    • Early APBI trials demonstrate excellent local control rates.
    • Good-to-excellent cosmetic outcomes are associated with APBI.
    • APBI potentially overcomes logistical barriers of WBI, increasing access to breast-conserving therapy.

    Conclusions:

    • APBI is a viable alternative to WBI for selected early-stage breast cancer patients.
    • Ongoing research and trials are crucial for long-term outcome data and refining APBI protocols.
    • Further investigation into APBI patient selection and delivery modalities is warranted.
