Area of Science:

  • Minimally Invasive Surgery
  • Surgical Education
  • Robotic Surgery

Background:

  • Maintaining surgical efficiency during trainee integration is a significant challenge.
  • Graduated surgical responsibility programs aim to balance trainee learning with operative efficiency.

Purpose of the Study:

  • To test the hypothesis that graduated surgical responsibility for trainees does not hinder operative efficiency.
  • To compare operative times between robotic procedures performed by trainees and attending surgeons.

Main Methods:

  • Retrospective cohort study of robotic cases (September 2008 - August 2011).
  • Trainee performance times were collected prospectively.
  • Comparison of overall and component operative times for hysterectomy and sacrocolpopexy.

Main Results:

  • Total robot docking time was longer for trainees (155 min) vs. attendings (132 min, p=0.011).
  • Mean performance times for hysterectomy (70 vs. 59 min, p=0.096) and sacrocolpopexy (76 vs. 79 min, p=0.545) were comparable.
  • Benchmark operative times were established for trainee hysterectomy and sacrocolpopexy steps.

Conclusions:

  • Robotic operative efficiency is comparable when trainees participate as console surgeons within a graduated responsibility program.
  • Trainee involvement does not negatively impact efficiency for specific robotic procedures.
