This case report details a rare aortic dissection variant, intimo-intimal intussusception, in a young male. Surgical repair of the ascending aorta was successful, highlighting a unique presentation of this critical cardiovascular condition.
Area of Science:
- Cardiovascular Surgery
- Thoracic Surgery
- Aortic Diseases
Background:
- Acute Stanford type A aortic dissection is a life-threatening condition.
- Intimo-intimal intussusception is a rare and poorly understood variant of aortic dissection.
- Early diagnosis and surgical intervention are crucial for patient survival.
Observation:
- A 38-year-old male presented with sudden back pain and unconsciousness.
- Computed tomography (CT) revealed intimal flaps in the aortic root and arch, but not the ascending aorta, with brachiocephalic artery occlusion.
- Intraoperative findings showed complete intimal detachment at the sinotubular junction with an inverted flap intruding into the brachiocephalic artery.
Findings:
- The patient underwent successful ascending aorta replacement under circulatory arrest.
- The rare intimo-intimal intussusception was confirmed during surgery.
- Postoperative recovery was uneventful.
Implications:
- This case expands the understanding of aortic dissection presentations.
- Highlights the importance of advanced imaging and surgical visualization for rare intimal flap abnormalities.
- Emphasizes the efficacy of surgical repair in managing complex aortic dissections.
