Summary

Children with isolated optic neuritis (ON) face a high risk of developing multiple sclerosis (MS). Cranial MRI (cMRI) abnormalities are a strong predictor of MS development, even after accounting for other factors.

Background:

  • Isolated optic neuritis (ON) is a common initial symptom of multiple sclerosis (MS).
  • Predicting MS conversion in pediatric ON patients is crucial for early intervention.

Purpose of the Study:

  • To investigate the risk of MS development in children after isolated ON.
  • To evaluate the predictive performance of cranial magnetic resonance imaging (cMRI), visual evoked potentials (VEPs), and oligoclonal bands (OCB) for MS development.

Main Methods:

  • Retrospective analysis of 34 pediatric patients with isolated ON.
  • Assessed predictors including age, sex, VEPs, ON type, cMRI, OCB, and visual recovery.
  • Utilized logistic regression models to identify significant predictors of MS progression.

Main Results:

  • Abnormal cMRI significantly increased the odds of MS development (OR 20.57, P < 0.001).
  • Positive OCB also increased MS development odds (OR 12.0, P = 0.001).
  • cMRI remained the sole statistically significant predictor in multiple regression analysis.

Conclusions:

  • cMRI lesions suggestive of MS and OCB are valuable in assessing MS progression risk after isolated ON.
  • Children with both cMRI abnormalities and positive OCB at ON onset are at high risk for developing MS.
