Bacterial signaling can occur within bacteria (intracellular) or between bacteria (intercellular). At times, a group of bacteria behaves like a community. Background:

  • Poly Implant Prothèse (PIP) mammary prostheses have a suspected high premature device failure rate.
  • A cohort study was designed to evaluate rupture prevalence, media impact, and ultrasound scan (USS) accuracy in PIP implant patients.

Purpose of the Study:

  • To re-evaluate the rupture prevalence of Poly Implant Prothèse (PIP) mammary implants.
  • To assess the effect of global media attention on patient recall and follow-up.
  • To determine the accuracy of ultrasound scans (USS) in detecting PIP implant ruptures.

Main Methods:

  • Follow-up of 460 patients with PIP silicone prostheses since their 2010 withdrawal.
  • Analysis of patient contact and review rates following media publicity.
  • Review of 283 patients, including breast ultrasound scans (USS) for 167, and surgical exploration for 85.

Main Results:

  • Kaplan-Meier analysis estimates 10-year PIP implant survival between 60-81%.
  • Occult device failure was found in 31.6% of post-publicity evaluees.
  • Ultrasound scan (USS) demonstrated high accuracy with 97.3% sensitivity and 93.1% specificity for rupture detection.

Conclusions:

  • PIP implants exhibit higher rupture prevalence compared to other breast prostheses.
  • Media coverage significantly improved patient recall and awareness of potential device failure.
  • A substantial proportion of patients with ruptured PIP devices were previously unaware, highlighting the importance of diagnostic imaging like USS.
