Summary
Adding pamidronate (a bisphosphonate) to surgical cement did not negatively impact its biomechanical properties. This finding supports further in vivo studies on bisphosphonate-enriched bone cement for enhanced bone quality.
Area of Science:
- Biomaterials Science
- Orthopedic Surgery
- Pharmacology
Background:
- Bisphosphonates (BPs) enhance bone mineral density and quality.
- Surgical cement requires biocompatibility and biotolerance.
Purpose of the Study:
- To evaluate the biomechanical properties of bone cement enriched with pamidronate.
- To compare these properties with clean bone cement.
Main Methods:
- Compressive and three-point flexural tests were performed on clean and pamidronate-enriched bone cement.
- Biomechanical parameters including stress, strain, and density were analyzed.
Main Results:
- No significant differences were observed in stress at maximal load, elastic region energy storage, or sample density.
- Enrichment with bisphosphonates induced yielding in the bone cement material.
Conclusions:
- Pamidronate-enriched bone cement retains favorable biomechanical properties.
- Further in vivo research is warranted to assess the performance of BP-enriched bone cement in bone implantation.

The Bone Matrix
Bone contains a relatively small number of cells entrenched in a matrix of collagen fibers that provide an adherent surface for inorganic salt crystals. Both components of the matrix, organic and inorganic, contribute to the unusual properties of bone. Without collagen, bones would be brittle and shatter easily. Without mineral crystals, bones would flex and provide little support. This can be observed by an experiment: when the minerals of a bone are dissolved by soaking the bone in acid or...
Strength of Cement
Strength tests for cement are not performed directly on neat cement paste due to difficulty in obtaining consistent, reliable specimens. Instead, cement is typically tested in the form of cement-sand mortar.
For compressive strength tests, ASTM C 109-05 standards prescribe a cement-sand mix ratio of 1:2.75 and a water/cement ratio of 0.485 for making 2-inch cubes. These cubes are mixed, cast, and cured in saturated lime water at 23°C until testing. Flexural strength testing, outlined in ASTM C...
