Background:

  • The pathogenesis of thyroid carcinoma in Graves' goiter remains unclear.
  • Preoperative diagnostic methods for these carcinomas are not well-established.

Purpose of the Study:

  • To investigate the incidence, clinical features, and pathological findings of thyroid carcinoma in patients with Graves' goiter.

Main Methods:

  • A retrospective study of 739 patients with Graves' disease undergoing subtotal thyroidectomy between October 1983 and September 1985.
  • Inclusion of an additional 4 patients with a history of Graves' disease who underwent surgery for thyroid carcinoma.
  • Preoperative roentgenography of the neck was performed on all patients.

Main Results:

  • Thyroid carcinoma was found in 2.0% (15/739) of resected specimens from Graves' disease patients.
  • The overall incidence of thyroid carcinoma associated with Graves' disease was 2.6% (19/743).
  • Histologically, 15 papillary and 4 follicular carcinomas were identified.
  • Carcinoma foci averaged 13.8 mm, with 52.6% measuring 10 mm or greater.
  • Invasive growth and lymph node metastasis were predominant.
  • Preoperative roentgenography detected calcification in 26.3% (5/19) of cases.

Conclusions:

  • Thyroid carcinoma in Graves' goiter can exhibit marked invasiveness and lymph node metastasis, even with small primary tumors.
  • Detection of calcification via roentgenography may aid in diagnosing carcinoma, especially when palpation is difficult in diffusely enlarged Graves' goiters.
