Summary

Becker muscular dystrophy patients with heart failure can benefit from biventricular cardioverter-defibrillators (CRT-D). This cardiac resynchronization therapy improved a patient's heart function and symptoms within one month.

Background:

  • Becker muscular dystrophy (BMD) can lead to non-ischemic dilated cardiomyopathy.
  • Patients may experience worsening heart failure (HF) and presyncope.
  • Left bundle branch block (LBBB) and mechanical dyssynchrony are common in these patients.

Observation:

  • A 44-year-old male with BMD and non-ischemic dilated cardiomyopathy presented with worsening heart failure (NYHA class III) and presyncope.
  • Echocardiography revealed left ventricular (LV) dilatation, hypertrabeculation, severely depressed systolic function (EF 20%), and significant mechanical dyssynchrony.

Findings:

  • A biventricular cardioverter-defibrillator (CRT-D) was implanted with leads positioned for optimal cardiac resynchronization.
  • Echocardiography-guided programming of the CRT-D device was performed to enhance atrio-, inter-, and intraventricular synchrony.

Implications:

  • CRT-D implantation led to substantial clinical improvement in the patient within one month post-implantation.
  • This case demonstrates the efficacy of CRT-D therapy in managing advanced heart failure secondary to Becker muscular dystrophy.
