Purpose of the Study:

  • To identify specific biomarkers in patients with COPD that can predict impending exacerbations.
  • To differentiate between biomarkers present during stable phases versus those indicating an oncoming exacerbation.

Main Methods:

  • Forty-three stable COPD patients were monitored for six months for exacerbations.
  • Sputum was collected for biomarker analysis (Leukotriene B4, 8-isoprostane, myeloperoxidase, IL-8) and purulence assessment.
  • Exhaled breath condensate (EBC), spirometry, impulse oscillometry, and quality-of-life questionnaires were also utilized.

Main Results:

  • Leukotriene B4 in sputum was significantly elevated prior to exacerbation compared to stable phases (p=0.05).
  • Tendencies for increased sputum 8-isoprostane, myeloperoxidase, and IL-8 were observed before exacerbation, with further increases during exacerbation.
  • Sputum purulence increased significantly at exacerbation (p=0.02), but EBC biomarkers, spirometry, and quality-of-life scores did not predict exacerbations.

Conclusions:

  • Sputum biomarkers, especially leukotriene B4, show potential as early predictors of COPD exacerbations.
  • Early identification of exacerbations via sputum biomarkers could facilitate prompt treatment.
  • This approach may help prevent disease progression and improve patient outcomes in COPD management.
