Background:

  • Quadripolar leads provide 10 pacing configurations, exceeding traditional bipolar leads.
  • These configurations may influence hemodynamic parameters and cardiac output (CO).

Purpose of the Study:

  • To evaluate if quadripolar lead pacing vectors can enhance cardiac output (CO).
  • To compare CO across various quadripolar pacing configurations and traditional methods.

Main Methods:

  • Cardiac output (CO) was measured using ECHO in 51 patients within 7 days of device implantation.
  • Pacing was assessed in "no-pacing" and all 10 quadripolar configurations, distinguishing traditional (tip-ring, tip-RVC, ring-RVC) and nontraditional vectors.
  • The optimal configuration maximized CO while maintaining a capture threshold <3 V and adequate safety margin.

Main Results:

  • The mean best CO with traditional quadripolar vectors (4.16 L/min) and all configurations (4.33 L/min) surpassed baseline non-paced CO (3.64 L/min).
  • The best quadripolar lead configuration yielded higher CO than the best traditional configuration.
  • Nontraditional vectors unique to quadripolar leads achieved the highest CO in 53% of patients.

Conclusions:

  • Quadripolar leads offer enhanced pacing capabilities for acute CO augmentation.
  • These leads provide superior options for optimizing cardiac function compared to conventional bipolar leads.
