Early T-cell precursor (ETP) leukaemias are a distinct, aggressive form of T-cell acute lymphoblastic leukaemia (T-ALL) with a poor prognosis. These leukaemias share genetic features with both T-ALL and acute myeloid leukaemia (AML), necessitating novel therapeutic strategies.
Area of Science:
- Hematology
- Oncology
- Molecular Biology
Background:
- Early T-cell precursor (ETP) leukaemias represent a recently identified subtype of T-cell acute lymphoblastic leukaemia (T-ALL).
- ETP T-ALL is characterized by a poor prognosis, highlighting the need for further research into its unique biological features.
Purpose of the Study:
- To review recent advancements in understanding the biology and genetics of ETP T-ALL.
- To explore the prognostic significance of ETP T-ALL.
- To identify potential new therapeutic strategies for this aggressive leukaemic subtype.
Main Methods:
- Immunophenotypic analyses to define ETP T-ALL.
- Transcriptional profiling to compare ETP T-ALL with other leukaemic subtypes.
- Genetic analyses to identify characteristic mutations and lesions.
Main Results:
- ETP T-ALL is immunophenotypically distinct and associated with a poor prognosis.
- Transcriptionally, ETP T-ALLs resemble haematopoietic stem cells and myeloid progenitors.
- ETP T-ALLs exhibit fewer typical T-ALL genetic lesions (e.g., CDKN2A/B deletions, NOTCH1 mutations) and more AML-associated mutations.
Conclusions:
- ETP T-ALL and early immature T-ALLs show a very early differentiation arrest with unique genetic and transcriptional profiles.
- These leukaemias bridge characteristics of both T-ALL and AML.
- The distinct biology and poor prognosis of ETP T-ALL underscore the urgent need for tailored therapeutic approaches.
