Summary

Recurrent sinusitis (RS) patients show reduced nasal mucosal secretions to cholinergic stimulation. This reversible deficiency in glandular antimicrobial factors may predispose individuals to recurrent infections.

Background:

  • Recurrent sinusitis (RS) is a prevalent condition with often unknown causes.
  • Investigating nasal mucosal responses may reveal underlying mechanisms.
  • Glandular antimicrobial factors are crucial for mucosal defense.

Purpose of the Study:

  • To assess nasal mucosal secretory responses in patients with recurrent sinusitis.
  • To determine if a deficiency in glandular antimicrobial factors contributes to RS.
  • To evaluate the reversibility of altered secretory responses.

Main Methods:

  • Compared nasal washings from 14 RS patients and 24 normal controls (NC).
  • Analyzed protein content (albumin, IgG, IgA, lactoferrin, lysozyme) after methacholine or histamine challenge.
  • Re-evaluated responses after medical treatment in a subset of RS patients.

Main Results:

  • RS patients had blunted cholinergic responses with decreased secretion of albumin, IgG, nonsecretory IgA (nsIgA), secretory IgA (sIgA), and lysozyme (LZM).
  • Baseline lactoferrin (LFN) and LZM were higher in RS patients.
  • Histamine responses were similar between RS and NC groups.
  • Abnormal cholinergic responses improved after medical treatment in RS patients.

Conclusions:

  • Patients with RS exhibit a reversible reduction in nasal mucosal secretory responses to cholinergic stimulation.
  • Impaired secretion of glandular proteins, rich in antimicrobial factors, may predispose to recurrent infections.
  • Targeting cholinergic pathways could be a therapeutic strategy for RS.
