The Cochlea

The cochlea is a coiled structure in the inner ear that contains hair cells—the sensory receptors of the auditory system. Sound waves are transmitted to the cochlea by small bones attached to the eardrum called the ossicles, which vibrate the oval window that leads to the inner ear. This causes fluid in the chambers of the cochlea to move, vibrating the basilar membrane.

    • Two novel cases of cochlear-facial dehiscence are presented.
    • Computed tomography (CT) revealed bony dehiscence between the facial nerve and cochlea in both patients.

    Findings:

    • Cochlear-facial dehiscence presents as a specific type of otic capsule dehiscence.
    • This condition can manifest as a "third-window lesion," causing characteristic auditory and vestibular symptoms.

    Implications:

    • Cochlear-facial dehiscence should be considered in the differential diagnosis of third-window lesions.
    • When superior canal dehiscence is absent on CT, clinicians should evaluate for cochlear-facial dehiscence in patients with suggestive symptoms.
