This study reports the first Korean case of onychomycosis, a nail infection, caused by the mold Chaetomium globosum. Diagnosis was confirmed through various methods, including DNA sequencing, and the patient received standard antifungal treatment.

Background:

  • Onychomycosis, a common nail fungal infection, is typically caused by dermatophytes.
  • Nondermatophytic molds and yeasts can also cause onychomycosis, expanding the spectrum of causative agents.
  • The genus Chaetomium comprises dematiaceous nondermatophytic molds with saprophytic lifestyles.

Observation:

  • A 35-year-old male presented with brownish-yellow discoloration and subungual hyperkeratosis in multiple toenails.
  • Microscopic examination revealed septate hyphae, and cultures on Sabouraud's dextrose agar (SDA) without cycloheximide showed characteristic mold growth.
  • No growth occurred on SDA with cycloheximide, and slide cultures displayed brown septated hyphae, perithecia, and ascospores.

Findings:

  • This case represents the first documented instance of onychomycosis caused by Chaetomium globosum in Korea.
  • Morphological analysis (KOH mount, slide culture) and DNA sequencing of the internal transcribed spacer region confirmed the causative agent as C. globosum.
  • Genetic analysis showed a 100% match to the reference strain ATCC 6205.

Implications:

  • This finding broadens the etiological understanding of onychomycosis, highlighting the role of nondermatophytic molds like Chaetomium.
  • Accurate identification of the causative agent is crucial for effective treatment of onychomycosis.
  • The successful treatment with oral terbinafine and topical amorolfine suggests standard antifungal therapies are effective against C. globosum onychomycosis.
