Background:

  • Previous reports have suggested an association between HC and leprosy, though the nature of this link remains unclear.

Observation:

  • A 61-year-old woman with borderline leprosy presented with facial lesions and impaired sensation.
  • Concurrently, she developed daily, continuous, unilateral headaches matching HC criteria, located on the same side as the facial lesions.
  • The headaches were severe, throbbing, and accompanied by ipsilateral conjunctival injection and lacrimation, significantly improving with indomethacin.

Findings:

  • The patient's presentation fulfills the diagnostic criteria for hemicrania continua.
  • The co-occurrence of HC and leprosy in this case supports a potential association between the two conditions.
  • Histopathology confirmed a borderline leprosy form, indicating active infection.

Implications:

  • This case highlights a potential link between leprosy and trigeminal autonomic cephalgias, specifically hemicrania continua.
