Diabetic Retinopathy01:27

Diabetic Retinopathy

DefinitionDiabetic retinopathy is a microvascular complication of diabetes affecting the retinal blood vessels.Risk FactorsDiabetic retinopathy is present in almost all individuals with type 1 diabetes and more than 60% of those with type 2 diabetes after two decades of disease. The risk increases with poor glycemic control, hypertension, dyslipidemia, smoking, pregnancy, and puberty.Although cataracts and glaucoma are also more frequent in people with diabetes, retinopathy remains the leading...
Pulmonary Tuberculosis III01:31

Pulmonary Tuberculosis III

Tuberculosis (TB) is a contagious infection primarily affecting the lung parenchyma but which can also affect other body parts. TB can be classified based on disease development, presentation, and the affected anatomical site.
The first classification is based on the development of the disease, and it includes the following categories:

Summary

Intraocular tuberculosis (TB) can cause retinal vasculitis and vision loss. Early diagnosis and antituberculosis treatment (ATT) significantly improved vision and prevented recurrences in a patient with Eales Disease.

Area of Science:

  • Ophthalmology
  • Infectious Diseases
  • Medical Microbiology

Background:

  • Intraocular tuberculosis (TB) presents varied clinical manifestations, notably retinal vasculitis, often affecting veins and leading to hemorrhages and neovascularization.
  • Diagnosis can be challenging, relying on clinical signs and systemic TB evidence.

Observation:

  • A 61-year-old woman experienced 6 years of blurred vision and floaters, diagnosed initially as retinal vein occlusion.
  • Bilateral venous occlusion and vasculitis suggestive of Eales Disease were observed.
  • Interferon gamma release assay (IGRA-QuantiFERON-TB Gold) confirmed TB infection.

Findings:

  • Antituberculosis treatment (ATT) was initiated.
  • The patient's best-corrected visual acuity improved from 20/50 to 20/20 bilaterally.

  • No recurrences were noted during a 15-month follow-up period.
  • Implications:

    • Antituberculosis treatment (ATT) effectively manages intraocular tuberculosis presenting as Eales Disease.
    • ATT can prevent recurrent vitreous hemorrhages and improve visual outcomes in affected individuals.
