    Background:

    • Congenital habitual patellar dislocation is a rare knee condition characterized by permanent, irreducible patellar dislocation present at birth.
    • It is associated with a short quadriceps, significant patellofemoral dysplasia, and short stature.
    • This condition often presents with genu valgus and lateral joint pain.

    Observation:

    • A 27-year-old woman presented with recurring bilateral habitual patellar dislocation despite previous failed realignment surgeries.
    • Clinical examination revealed genu valgus knees and persistent lateral joint pain.
    • Radiographs confirmed bilateral patellar dislocation, genu valgum, patellofemoral dysplasia, and lateral compartment osteoarthritis.

    Findings:

    • Preoperative standing radiographs indicated significant valgus deformities (anatomic: R 13°, L 6°; mechanical: R 8°, L 2°) and altered weight-bearing lines (R 65%, L 48%).
    • The surgical intervention involved a distal femoral closing wedging osteotomy to correct the valgus deformity.
    • Simultaneously, percutaneous lateral release and medial reefing were performed to stabilize the patellas bilaterally.

    Implications:

    • This combined surgical approach effectively addressed both the underlying valgus deformity and patellar instability in a complex case.
    • The successful management highlights a potential treatment strategy for recurrent patellar dislocations secondary to significant genu valgum and dysplasia.
    • Further research could explore long-term outcomes and compare this technique with other surgical options for congenital patellar instability.
