Cytokine gene polymorphisms in kidney transplantation.

Cytokine gene polymorphisms, including tumor necrosis factor-alpha and transforming growth factor-beta1, do not predict kidney transplant rejection alone. However, specific haplotypes are linked to acute rejection episodes and poorer graft survival.

- Analyzed single nucleotide polymorphisms (SNPs) for TNF-α, TGF-β1, IL-10, IL-6, and IFN-γ in 231 kidney transplant recipients (KTR).

- Included KTR treated with mycophenolate mofetil (MMF+) and those without (MMF-).

- No single polymorphism was significantly associated with acute rejection (ARE) or chronic allograft dysfunction (CAD).
- TGF-β1 Low production showed a trend towards better graft survival (81.8% at 20 years) but was not significant (P = .2).
- In MMF- KTR without HLA-mismatches or pre-formed antibodies, the TNF-α*H/TGF-β1*H/IL-10*H haplotype was associated with significantly higher ARE (75%) compared to others (16%; P = .03).
Conclusions:
- The TGF-β1 High secretion profile may offer kidney graft protection.
- The TNF-α*H/TGF-β1*H/IL-10*H haplotype is linked to increased ARE risk and diminished graft survival.
