Insights

Pediatric stroke, though rare, requires consideration for focal neurological deficits. Methylenetetrahydrofolate reductase (MTHFR) C677T heterozygosity was identified as a potential factor in this case.

Area of Science:

  • Neurology
  • Pediatrics
  • Genetics

Background:

  • Childhood stroke is an increasingly recognized cause of significant morbidity and mortality.
  • Typical presentations include acute focal neurological deficits, headache, altered consciousness, or seizures.

Observation:

  • An 18-month-old male presented with motor developmental delay, gait disturbance, and right hemiparesis with left arm preference.
  • Brain MRI revealed extensive old ischemic injury in the left middle cerebral artery territory, affecting the corona radiata, thalamus, internal capsule, and lenticular nucleus.

Findings:

  • Causal investigation identified methylenetetrahydrofolate reductase (MTHFR) C677T heterozygosity.
  • The child received physiotherapy and pediatric neurology consultation, showing favorable progress without recurrent ischemic events.

Implications:

  • Pediatric stroke should be considered in cases of focal neurological deficits.
  • While MTHFR C677T mutation is a prothrombotic factor, direct therapeutic intervention is not necessary in the absence of hyperhomocysteinemia.

Autism Spectrum Disorder

Autism spectrum disorder (ASD) is a neurodevelopmental condition marked by persistent deficits in social communication and interaction alongside restrictive and repetitive behaviors or interests. ASD is sometimes accompanied by intellectual impairment.
These core symptoms manifest differently among individuals, ranging from mild to severe. The disorder's complexity extends beyond its clinical presentation, encompassing a diverse range of biological, cognitive, and sociocultural influences.

Alterations in Muscle Tone ll

Alterations in muscle tone are common manifestations of neurological disorders and reflect dysfunction within different nervous system regions. Spasticity, paratonia, and dystonia represent distinct forms of hypertonia, each with unique mechanisms, clinical features, and diagnostic importance.CharacteristicsSpasticity happens from upper motor neuron lesions and is characterized by velocity-dependent resistance to passive movement. Clinical features include:Exaggerated deep tendon reflexesClonus...
Stroke: Introduction and Types01:29

Stroke: Introduction and Types

A stroke is an acute neurological event caused by the sudden disruption of cerebral blood flow, leading to rapid loss of neuronal function. Neurons depend on continuous oxygen and glucose supply, so even brief interruptions can cause irreversible injury within minutes. Strokes are classified into ischemic and hemorrhagic types.Ischemic StrokeIschemic strokes are most common and occur due to arterial occlusion, depriving brain tissue of oxygen and nutrients. This leads to energy failure, ionic...
