Summary

Mesenteric fibromatosis is more aggressive in Gardner's syndrome patients, with a higher recurrence risk. Complete surgical excision may not always be necessary for these tumors.

Background:

  • Fibromatosis, also known as desmoid tumors, can occur in the mesentery and peritoneum.
  • Gardner's syndrome is associated with an increased risk of various tumors, including fibromatosis.

Purpose of the Study:

  • To investigate features associated with recurrence in mesenteric and peritoneal fibromatosis.
  • To compare recurrence rates and outcomes between patients with and without Gardner's syndrome.

Main Methods:

  • Retrospective study of 82 cases of mesenteric and peritoneal fibromatosis.
  • Analysis of surgical outcomes, recurrence rates, and patient demographics, including presence of Gardner's syndrome.

Main Results:

  • Complete excision led to no recurrence in 56 of 72 patients.
  • Patients with Gardner's syndrome had a significantly higher risk of recurrence and related mortality.
  • Pathologic features, trauma history, estrogen exposure, size, and mitotic activity did not correlate with recurrence.

Conclusions:

  • Mesenteric fibromatosis is more aggressive in Gardner's syndrome patients.
  • Pathologic features are poor predictors of fibromatosis behavior.
  • Complete surgical excision may not be universally required for mesenteric fibromatosis.
