Summary

Atypical benign partial epilepsy (ABPE) involves distinct neuronal network patterns. This study reveals overlapping characteristics with other childhood epilepsy syndromes, suggesting a spectrum of conditions.

Background:

  • Atypical benign partial epilepsy (ABPE) is a specific type of childhood idiopathic focal epilepsy.
  • Understanding the underlying neuronal networks is crucial for classifying and treating these syndromes.
  • Previous electroencephalography-functional magnetic resonance imaging (EEG-fMRI) studies have explored related epilepsy types.

Purpose of the Study:

  • To investigate the neuronal networks associated with ABPE.
  • To compare EEG-fMRI findings in ABPE with those in related childhood epilepsy syndromes.

Main Methods:

  • Simultaneous EEG-fMRI recording was performed on ten patients diagnosed with ABPE.
  • Interictal epileptiform discharges (IEDs) were recorded and analyzed for associated blood oxygen level-dependent (BOLD) signal changes.
  • Individual subject and group analyses were conducted to identify focal and distant BOLD signal alterations.

Main Results:

  • IED-associated BOLD signal changes were detected in most recordings.
  • Both focal BOLD signal changes (concordant with spike fields) and distant cortical/subcortical changes were observed.
  • Group analysis identified thalamic activation, and findings showed similarities to rolandic epilepsy and continuous spikes and waves during slow sleep (CSWS).

Conclusions:

  • ABPE exhibits neuronal network patterns that overlap with other idiopathic focal epilepsies of childhood.
  • Findings suggest that these childhood epilepsy syndromes exist on a spectrum with shared characteristics.
  • EEG-fMRI is a valuable tool for elucidating the neurobiological underpinnings of these epilepsy subtypes.
