Microfractures at the rotator cuff footprint may improve short-term pain after repair. However, this technique does not show significantly different long-term clinical or imaging outcomes compared to traditional rotator cuff repair.

Area of Science:

  • Orthopedic Surgery
  • Sports Medicine
  • Regenerative Medicine

Background:

  • Rotator cuff tears are common injuries requiring surgical repair.
  • Enhancing tendon healing at the bone-tendon junction is crucial for successful outcomes.
  • Growth factors released from microfractures may promote tendon healing.

Purpose of the Study:

  • To evaluate the efficacy of microfractures at the footprint in enhancing rotator cuff tendon healing.
  • To compare clinical and functional outcomes of rotator cuff repair with and without microfracture treatment.

Main Methods:

  • A randomized controlled trial involving 57 patients undergoing arthroscopic rotator cuff repair.
  • Patients were divided into two groups: one receiving microfracture at the footprint, the other serving as a control.

  • All patients followed the same standardized post-operative rehabilitation protocol.
  • Main Results:

    • Both groups showed significant improvement from baseline at a minimum two-year follow-up.
    • At three months, the microfracture group demonstrated significantly better visual analogue scale (VAS), range of motion (ROM), and University of California at Los Angeles (UCLA) and Constant scores.
    • At the final follow-up (minimum two years), no significant inter-group differences in clinical or functional outcomes were observed.

    Conclusions:

    • Microfractures at the rotator cuff footprint are a safe, simple, and cost-effective adjunct to repair.
    • This technique leads to reduced short-term pain after rotator cuff repair.
    • Microfractures do not provide significant long-term clinical or imaging benefits compared to traditional rotator cuff repair.
