Background:

  • Inflammatory myopathies, such as polymyositis, can affect multiple organ systems.
  • Peripheral muscle weakness in polymyositis may mask underlying cardiac and pulmonary dysfunction.

Purpose of the Study:

  • To investigate cardiac and pulmonary function in polymyositis patients.
  • To determine if cardiac and pulmonary impairments are masked by peripheral muscle weakness.

Main Methods:

  • 11 polymyositis patients underwent pulmonary function tests (PFTs), echocardiography/Doppler studies, and exercise testing.
  • Evaluated maximal respiratory pressures, expiratory flow rates, lung volumes, diffusion capacity (Dsb), aerobic capacity (VO2 max), O2 pulse, and heart rate response.

  • Most respiratory pressures and flow rates were normal. One patient had restrictive lung disease; two had reduced Dsb.
  • Seven patients showed evidence of pulmonary hypertension. Aerobic capacity was impaired in 8/11 patients (VO2 max 46% of predicted).
  • Seven patients had a reduced O2 pulse, and five exceeded 85% of their target heart rate during exercise.

Conclusions:

  • Asymptomatic pulmonary function impairments are uncommon in polymyositis.
  • Pulmonary hypertension is prevalent in polymyositis patients.
  • Peripheral muscle weakness can mask cardiac and pulmonary symptoms, leading to underdiagnosis.
