Summary

Lymph node density is a significant predictor of overall survival in head and neck cancer patients. Lower lymph node density (≤0.20) is associated with better outcomes and longer time to treatment failure.

Lymph nodes are bean-shaped structures that cluster along the lymphatic vessels in the inguinal, axillary, and cervical regions. Each node is divided into compartments by a capsule that extends trabeculae inward.

Main Results:

  • Lymph node density ≤0.20 was associated with significantly improved overall survival (79% vs 50%, p = .04).
  • A trend towards improved 3-year locoregional control and distant metastasis-free survival was observed with lower lymph node density.
  • Patients with lymph node density >0.20 experienced earlier treatment failure (median, 5.2 months vs 12.7 months).

Conclusions:

  • Lymph node density is a valuable prognostic indicator for overall survival in head and neck cancer.
  • Differences in overall survival may be attributed to variations in the time to treatment failure based on lymph node density.
