Background:

  • Ring melanoma, a rare malignant melanoma infiltrating over 180 degrees of the ciliary body, is infrequently observed in Japan.
  • This report details a case identified during the management of traumatic glaucoma.

Observation:

  • A 44-year-old woman presented with persistent high intraocular pressure (30 mmHg) in her left eye following blunt trauma.
  • Gonioscopy revealed approximately 180 degrees of angle recession, and anti-glaucoma medication proved ineffective.
  • Ultrasound biomicroscopy (UBM) demonstrated progressive iris elevation and 360-degree ciliary body thickening.

Findings:

  • Atypical cells with melanin granules were detected in the aqueous fluid.
  • Positron Emission Tomography-Computed Tomography (PET-CT) yielded positive findings.
  • Histopathological examination confirmed the diagnosis of ring melanoma after enucleation.

Implications:

  • Ring melanoma should be considered in the differential diagnosis of intractable secondary glaucoma.
  • Early detection using advanced imaging like UBM is vital for managing rare ocular tumors.
