Summary

Primary HIV infection can present with flu-like symptoms. In rare cases, it can also manifest as Pneumocystis pneumonia, responding well to trimethoprim-sulfamethoxazole treatment.

Background:

  • Primary HIV infection (PHI) affects 40-90% of newly infected individuals.
  • PHI symptoms are variable, often flu-like, with high viral load and decreased CD4 count.

Observation:

  • A homosexual man presented with PHI symptoms and a non-productive cough.
  • Chest CT revealed diffuse ground glass opacity.
  • Recent HIV seroconversion was confirmed.

Findings:

  • Pneumocystis jirovecii pneumonia (PJP) was diagnosed via transbronchial lung biopsy.
  • Symptoms rapidly improved with trimethoprim-sulfamethoxazole treatment.

Implications:

  • Pneumocystis pneumonia, typically an AIDS-defining illness, can occur during primary HIV infection.
