Background:

  • Intermittent claudication (IC) is characterized by lower limb pain during exercise due to arterial insufficiency.
  • Remote ischemic preconditioning (RIPC) is a protective phenomenon involving ischemia in one organ conferring resistance to ischemia in another.

Purpose of the Study:

  • To investigate the effect of RIPC on walking ability in patients with intermittent claudication.
  • To determine if RIPC enhances muscle tissue resistance to ischemia in IC patients.

Main Methods:

  • 52 ambulatory patients with IC were recruited and randomly assigned to three groups (RIPC before second test, RIPC before first test, or control).
  • Participants underwent two treadmill tests (Gardener protocol) seven days apart.
  • RIPC was induced via sub-lethal ischemia in a limb contralateral to the symptomatic one.

Main Results:

  • Group A, which received RIPC before the second test, demonstrated a significant increase in initial claudication distance compared to their first test.
  • No significant difference was observed in total walking distance for any group.

Conclusions:

  • RIPC effectively increased the initial claudication distance in patients suffering from intermittent claudication.
  • RIPC did not demonstrate a significant impact on the overall walking distance achievable by these patients.
