A patient experienced spontaneous rupture of the extensor pollicis longus (EPL) tendon. Prophylactic decompression of the affected EPL tendon is recommended to prevent rupture in cases of tendinosis.

Background:
- Spontaneous rupture of the extensor pollicis longus (EPL) tendon is a rare but significant clinical event.
- Tendinosis of the EPL tendon can predispose it to rupture.
- Wrist pain and weakness are common symptoms associated with EPL tendinopathy.
Observation:
- A patient presented with spontaneous right EPL tendon rupture and left-sided wrist pain/weakness due to tendinosis.
- MRI confirmed tendinosis in the left EPL tendon.
Findings:
- Surgical decompression of the symptomatic left EPL tendon resolved pain and weakness.
- Reconstruction of the right EPL tendon was performed using a palmaris longus graft.
- Prophylactic decompression of the left EPL tendon successfully prevented contralateral rupture.
Implications:
- Ultrasound imaging is recommended for evaluating EPL tendinosis.
- Prophylactic decompression of EPL tendons with tendinosis may prevent spontaneous rupture.
- Early intervention in tendinopathy can avert severe tendon injuries.
