Background:

  • Retinoblastoma, a common intraocular malignancy in children, can present with orbital cellulitis.
  • Orbital cellulitis in retinoblastoma often indicates advanced disease with significant inflammation.

Purpose of the Study:

  • To evaluate the efficacy of pre-enucleation systemic steroids in mitigating inflammation associated with retinoblastoma-induced orbital cellulitis.
  • To assess the impact of steroids on the surgical management of these complex cases.

Main Methods:

  • Retrospective review of retinoblastoma patients presenting with orbital cellulitis over three years.
  • Analysis of clinical, radiological, and histopathological data.
  • Assessment of outcomes in patients who received pre-enucleation systemic steroids.

Main Results:

  • 14 cases of retinoblastoma-associated cellulitis were identified (5.39% of retinoblastoma cases).
  • Nine patients with orbital cellulitis and intraocular tumors occupying 80-100% of the globe were included.
  • Five patients receiving pre-enucleation steroids showed a prompt decrease in inflammation.

Conclusions:

  • Advanced, necrotic retinoblastoma with anterior segment involvement can mimic orbital cellulitis.
  • Pre-enucleation systemic steroids are beneficial in managing inflammation and facilitating surgical intervention for retinoblastoma presenting as orbital cellulitis.
