The role of right ventricular function in mitral valve surgery

Impaired right ventricular function before mitral valve surgery is linked to increased resource use and postoperative regurgitation. However, it does not impact short- or mid-term survival rates following the procedure.

Purpose of the Study:
- To determine the impact of preoperative right ventricular function on postoperative outcomes after mitral valve repair (MVP) or replacement (MVR).
Main Methods:
- 335 patients undergoing MVP or MVR between 2007-2012 were analyzed.
- Preoperative and postoperative echocardiography (transthoracic and transesophageal) assessed right ventricular function and MV performance.
- Patients were categorized into normal-to-mild (grade 1-2) and moderate-to-severe (grade 3-4) right ventricular dysfunction groups.
Main Results:
- 54 patients had moderate-to-severe right ventricular dysfunction (group 2) versus 280 with normal-to-mild dysfunction (group 1).
- Group 2 patients required more MVR, experienced higher rates of postoperative MV regurgitation, and had longer ICU and hospital stays.
- Operative mortality was similar between groups, but group 2 had increased blood product transfusion needs.
Conclusions:
- Preoperative right ventricular dysfunction is associated with increased resource utilization and higher rates of postoperative mitral valve regurgitation.
- Despite these factors, moderate-to-severe right ventricular dysfunction does not significantly affect 1-year survival rates after mitral valve surgery.
