Background:

  • Group-based weight loss programs are common for overweight/obese individuals with type 2 diabetes.
  • The impact of group dynamics and participant characteristics on weight loss outcomes is not well understood.

Purpose of the Study:

  • To investigate whether weight loss and adherence cluster within treatment groups.
  • To determine if group characteristics (size, homogeneity) influence weight loss outcomes in an intensive lifestyle intervention (ILI).

Main Methods:

  • Analysis of weight loss and adherence data (attendance, meal replacements, activity) over one year.
  • Examined 2329 ILI participants across 209 treatment groups in the multicenter Look AHEAD study.

Main Results:

  • Weight loss did not show significant clustering within groups (ICC=0.007).
  • Adherence measures exhibited small to moderate clustering (ICCs=0.05-0.11).
  • Neither group size nor baseline homogeneity affected weight loss outcomes, despite variations in group weight loss percentages.

Conclusions:

  • Controlling for clustering may not be essential in behavioral weight loss studies.
  • Group treatment alone is insufficient to leverage social influences for enhanced weight loss.
