Median sternotomy poses challenges for pediatric ectopia cordis. A staged Fontan procedure using a clamshell incision and sternothoracotomy approach offers a viable alternative for complex congenital heart defects.

- This technique may improve outcomes for children with ectopia cordis and associated anomalies undergoing staged palliation.
