Area of Science:

  • Orthopedic Surgery
  • Radiology
  • Biomedical Engineering

Background:

  • Femoral stem version is critical for total hip replacement (THR) outcomes, impacting dislocation risk.
  • Accurate measurement of femoral stem anteversion is essential for surgical planning and revision.
  • Current radiological methods for assessing stem anteversion are lacking.

Purpose of the Study:

  • To develop and validate a novel radiological method for measuring femoral stem anteversion.
  • To assess the reliability and validity of the proposed modified Budin view technique.

Main Methods:

  • A modified Budin view radiograph was employed in 36 THR patients.
  • Femoral stem anteversion was measured using the angle between the stem-neck axis and posterior intercondylar line.
  • Measurements were compared against CT scans for validity and assessed for intra- and interobserver reliability.

Main Results:

  • The modified Budin view demonstrated excellent intra- and interobserver reliability.
  • Radiological measurements showed strong correlation with CT measurements (r=0.877, p=0.001).
  • Mean radiological measurement (13.36°) closely approximated mean CT measurement (12.35°).

Conclusions:

  • The modified Budin view is a reliable and valid radiological method for measuring femoral stem anteversion.
  • This technique provides a practical tool for assessing stem version in THR.
  • Accurate anteversion measurement can aid in reducing dislocation risk and improving THR outcomes.
