Background:

  • Master cyclists, despite excellent cardiovascular fitness, can be at risk for coronary artery disease (CAD).
  • Familial hypercholesterolemia (FH) is a significant genetic risk factor for premature CAD.
  • Exercise stress testing is a common screening tool for CAD in athletes.

Observation:

  • An older brother (66) with asymptomatic ST depression on exercise stress test had multi-vessel CAD requiring bypass surgery, later completing a 600 km cycling tour.
  • A younger brother (59) with a normal exercise stress test suffered a cardiac arrest due to severe left main coronary artery stenosis, survived, and underwent intervention.

Findings:

  • Both brothers had multi-vessel CAD attributed to familial hypercholesterolemia.
  • A negative exercise electrocardiogram did not exclude advanced CAD in a high-risk individual.
  • Severe left main coronary artery stenosis was found in the younger brother despite normal stress test results.

Implications:

  • Asymptomatic individuals, including master athletes with a family history of CAD, may have significant coronary artery disease.
  • Routine exercise stress testing may not be sufficient to rule out advanced CAD in high-risk populations.
  • Further cardiovascular investigations are warranted in master athletes with a positive family history of CAD, even with normal stress test results.
