Background:

  • The preepiglottic space (PES) anatomical status is debated in laryngeal cancer TNM classifications.
  • Current TNM staging (UICC, AJCCS) implies T3 for tumors invading the PES.

Purpose of the Study:

  • To determine the significance of the preepiglottic space (PES) in laryngeal cancer progression.
  • To evaluate the statistical relationship between PES invasion and other cancer spread variables.

Main Methods:

  • Serial sectioning of 154 laryngeal specimens.
  • Statistical analysis of cancer spread variables using a computer program.

Main Results:

  • PES invasion frequently occurred with primary supraglottic tumors.
  • Over 50% of cases with PES invasion also showed base of tongue or strap muscle invasion.
  • PES invasion was noted in over 50% of transglottic cancer spread cases, all highly significant.

Conclusions:

  • PES invasion correlates strongly with advanced laryngeal cancer spread.
  • PES invasion should be considered equivalent to extralaryngeal spread (T4 staging).
  • Supraglottic partial laryngectomy may not be curative for tumors with PES invasion, except in very early stages.
