Summary

FDG-PET-CT scans accurately detect bone marrow involvement in diffuse large B-cell lymphoma (DLBCL) more effectively than biopsies. However, histological confirmation of bone marrow disease still impacts patient survival outcomes.

Area of Science:

  • Oncology
  • Nuclear Medicine
  • Hematology

Background:

  • Diffuse large B-cell lymphoma (DLBCL) is an aggressive non-Hodgkin lymphoma.
  • Accurate staging is crucial for effective treatment planning in DLBCL.
  • Bone marrow involvement (BMI) is a key factor in DLBCL staging and prognosis.

Purpose of the Study:

  • To compare the accuracy of FDG-PET-CT with bone marrow biopsy (BMB) for detecting BMI in newly diagnosed DLBCL patients.
  • To evaluate the prognostic significance of BMI identified by both methods on overall survival (OS) and progression-free survival (PFS).

Main Methods:

  • Retrospective analysis of newly diagnosed DLBCL patients.
  • Comparison of findings from FDG-PET-CT scans and iliac crest BMB.
  • Statistical analysis to assess accuracy and prognostic impact.

Main Results:

  • FDG-PET-CT demonstrated higher accuracy in identifying bone marrow involvement compared to BMB.
  • Histological confirmation of bone marrow involvement by BMB retained independent prognostic value for OS and PFS.
  • Discordant findings between imaging and histology were observed.

Conclusions:

  • FDG-PET-CT is a more sensitive tool for detecting bone marrow involvement in DLBCL.
  • Despite the accuracy of FDG-PET-CT, histological confirmation of bone marrow involvement remains prognostically significant.
  • Integrated interpretation of FDG-PET-CT and BMB may optimize DLBCL staging and prognostication.
