Patient-centered Care01:13

Patient-centered Care

Patient-centered care involves delivering care beyond inpatient hospitalization. Reflective practice can enhance a patient-centered approach. Reflective practice is a process of reasoning that considers all aspects of the present situation, including practicalities, learning from personal practice, and consideration of patient needs. Patients appreciate care decisions made while considering their input. Involving the patient in their care provides the patient with a sense of contribution rather...

Methods of Documentation VI: Case Management Model

The case management model is a multidisciplinary approach that involves healthcare professionals from diverse disciplines, such as physicians, nurses, therapists, social workers, and pharmacists, working collaboratively to address the various needs of patients. Each healthcare professional brings unique expertise and perspectives, contributing to a more comprehensive understanding of the patient's condition and tailoring treatment plans accordingly.
For example, a patient with a chronic illness...

Healthcare Associated Infections II: Preventive Measures

Essential infection prevention measures are based on the knowledge of the infection chain, the modes of transmission in healthcare settings, and the use of the best practices in all healthcare settings. Compulsory public reporting of healthcare-associated infection rates is needed to allow individuals and the community to make informed choices regarding selecting a healthcare facility.
The best practices for preventing healthcare-associated infections include hand hygiene, patient risk...

Primary Healthcare Services

Primary care promotes wellness and prevents disease. This care includes health promotion, education, protection (such as immunizations), early disease screening, and environmental considerations. Settings providing this type of healthcare include physician offices, public health clinics, school nursing, and community health nursing.
In 1978, international leaders convened in Alma-Ata, Kazakhstan, for what would be a pivotal event in global health. The Alma-Ata Declaration was the first to call...
Community Based Intervention01:30

Community Based Intervention

Community-based interventions in mental health represent a paradigm shift from institution-centered care to treatments embedded within the fabric of local communities. By prioritizing inclusion and leveraging existing societal structures, this approach fosters a supportive environment conducive to addressing mental health challenges while promoting individual dignity and agency.
Foundations of Community Mental Health Programs
Central to the success of community-based interventions is the...

Area of Science:

  • Health Services Research
  • Primary Care Medicine
  • Healthcare Quality Improvement

Background:

  • The Patient Centered Medical Home (PCMH) model aims to improve primary care delivery.
  • Understanding the impact of PCMH implementation on healthcare costs and quality is crucial for optimizing patient outcomes and resource allocation.

Purpose of the Study:

  • To examine the associations between partial and incremental implementation of the Patient Centered Medical Home (PCMH) model and measures of cost and quality of care.
  • To assess how different levels of PCMH adoption affect healthcare expenditures and the quality of services provided.

Main Methods:

  • Combined validated, self-reported PCMH capabilities data with administrative claims data for 2,432 primary care practices in Michigan.
  • Measured PCMH capabilities in place and changes implemented over a one-year period.
  • Used generalized estimating equations to estimate the effects of PCMH measures on total medical costs and quality of care, controlling for confounders.

Main Results:

  • Full PCMH implementation was associated with higher quality composite scores (3.5%) and preventive care scores (5.1%), and lower medical costs ($26.37 PMPM) for adults.
  • Full PCMH implementation also showed a significant increase in preventive care scores (12.2%) for pediatric populations, without cost reductions.
  • Incremental PCMH improvements positively impacted quality of care for both adult and pediatric populations but did not yield cost savings.

Conclusions:

  • The positive effects of the PCMH model on healthcare quality and cost appear to be dose-dependent, improving with the degree of implementation achieved.
  • Incremental advancements in PCMH implementation strategies are associated with enhanced quality of care for diverse patient populations.
