Summary

Early-stage idiopathic macular holes may resolve spontaneously with vitreous separation from the fovea. This natural process appears more beneficial than the risk of progression to a full macular hole.

Background:

  • Idiopathic macular holes are a common cause of visual impairment.
  • The natural history and prognostic factors of early-stage macular holes require further elucidation.

Purpose of the Study:

  • To investigate the prognosis of early-stage idiopathic macular holes.
  • To determine the relationship between vitreous condition and macular hole progression.

Main Methods:

  • Prospective follow-up study of 45 eyes with early-stage idiopathic macular holes.
  • Mean follow-up period of 32 months (range, 7 to 87 months).
  • Assessment of vitreous separation from the fovea at initial examination and during follow-up.

Main Results:

  • Vitreous separation from the fovea was observed in 18 eyes initially and developed in 10 eyes during follow-up (total 28 eyes).
  • None of the 28 eyes with vitreous separation progressed to a fully developed macular hole; 82% showed improvement.
  • Of the 27 eyes without vitreous separation, 37% progressed to a fully developed macular hole.

Conclusions:

  • Spontaneous vitreous separation from the fovea may lead to the reversal of early-stage idiopathic macular holes.
  • The chance of spontaneous resolution appears greater than the risk of progression to a full macular hole.
